Loss and Damage Claim Form Loss and Damage Claim FormImportant: Unless otherwise agreed in writing by an authorized representative of Aquabella (The Company), shipments travel at the Customer’s risk. The title passes to the Customer upon delivery to the Carrier. All claims for loss or damage in transit must be made to the delivering carrier by the Customer. In the event that The Company arranged the shipment on behalf of the Customer, the Customer should complete the Aquabella Freight Claim Form with supporting documentation and The Company will submit the claim on the Customer behalf. The Customer will be liable to pay The Company for all the material shipped and The Company will either credit the Customer’s account or issue a refund when the Carrier pays the claim to The
